Vital prognosis: engaged, reserved, what does it mean?

Vital prognosis is a medical term that designates the risks of death (or the chances of survival) of a patient in the event of illness or following an accident. The committed or reserved vital prognosis is mainly used in emergency situations.

Committed vital prognosis: definition

It is said that a person's vital prognosis is engaged if they are in critical condition and if they are at risk of succumbing. "It's a speech of a hospital doctor from the SAMU or an emergency doctor who communicates with the media" underlines Dr. Pascal Douek, general practitioner, nutritionist and sports doctor. "This term is used when a medical situation is not medically stable, the vital constants are not stabilized, the assessment of the lesions is not carried out and the situation can change at any time" indicates the doctor . In the event of a vital prognosis, the person concerned is taken to intensive care or to the operating room if an intervention must be carried out urgently.

Reserved vital prognosis: definition

The term "reserved vital prognosis" is identical to "committed vital prognosis". It is moreover this medical term which is rather used by the emergency doctors when they cannot decide on the survival of the patient. “The term reserved vital prognosis is a new word associated with vital urgency and used by the media” emphasizes the doctor.

The term vital prognosis is used when life is not in danger. "We are then in a situation in which the vital functions are not affected or are controlled" specifies Dr. Pascal Douek.

"The vital prognosis is no longer engaged when surgery or intensive care treatment has made it possible to control vital functions or when doctors have a precise understanding of the bodily damage that has been done and that it can be repaired" explains Dr. Pascal Douek.

Short-term vital prognosis (palliative care): significance

The term "short-term vital prognosis" is used when the end of life can happen at any time, when a person is in the terminal phase of a disease such as cancer. “Doctors never know how long it will last” underlines the doctor. A person suffering from a serious and incurable disease and whose prognosis is life-threatening in the short term, presenting with suffering that is refractory to treatment, can benefit from deep and continuous sedation, maintained until death.

Committed vital prognosis: can the evolution still be favourable?

"Vital prognosis engaged" does not mean that the person is going to die. The situation can evolve towards death but also fortunately towards a vital prognosis which is no longer engaged. “This will, for example, be the case when surgery has stopped a massive hemorrhage or when we have succeeded in controlling a severely impaired cardiac or cerebral function” explains Dr. Pascal Douek.
