What to do when you still have nausea in the 1st trimester of past pregnancy?

Like 80 % of pregnant women, you are prone to nausea or vomiting (called NVG) since the start of your pregnancy?Rest assured, this is completely normal."Both are considered by 99.4 % of the latter as the most invalidating symptom among early pregnancy evils" warns our expert, Rachel Halimi. They generally start between 4 and 6 weeks of amenorrhea (SA) and can persist around 14 SA (end of the first quarter). These rarely continue after the 20th SA.But what happens if it is the case?

The nausea of the 2nd and 3rd trimester

If nausea persists after the 1st trimester, there is no problem.Indeed, nausea and vomiting can last throughout pregnancy."Until the 3rd quarter, this concerns 23.5 % of patients" informs us Rachel Halimi. In addition, nausea can disappear at the end of the 1st trimester and then reappear shortly before childbirth (in the 8th or 9th month of pregnancy).

Like the nausea of the first quarter, the causes are unknown but probably multifactorial. As our midwife explains to us, are suspected:

- A hormonal cause: due to the high concentration of HCG (human chorionic gonadotrophin). This is the most incriminated hormone in NVG pathophysiology.High estrogens rates also play a role.

- Psycho-social factors: stress, fatigue, relationship problems, eating disorders and all the concerns that can be encountered in everyday life can have this impact on pregnant women.

- A mechanical cause: the uterus can compress the stomach, moreover, there is a relaxation of the gastroesophageal sphincter due to progesterone.

- A digestive cause: the stomach is weakened because it is subject to more acidity and this irritates the intestinal mucous membranes. It is also likely that a bacteria the Helicobacter pylori irritates the wall of the stomach and exacerbates vomiting.

- Vitamin B6 deficiency (pyridoxine): it is not known if it is the deficit that induces NVG or if a significant rate of vitamin B6 in certain patients who protect NVG.

When should you worry about nausea?

There are more serious inconclusive vomiting called hyperemèsis gravidarum (HG).They can lead to brief hospitalization due to significant weight loss (more than 5 %), dehydration, electrolytic disorders (a lack of minerals in the body, such as sodium, calcium or potassium),ketonuria (acute undernutrition) and vitamin deficiencies. These severe and incessant vomiting resist the treatments and occur throughout the day, including night, uncontrolled.Suffering women These problems can present themselves to exhausted, dehydrated and anxious obsolete emergencies. They encounter real difficulties in eating. This represents 0.5 % to 2 % of women.Let us add a more positive point, the spontaneous miscarriage rate would be lower during inconsistent vomiting.

What are the treatments for nausea?

Que faire quand on a encore des nausées le 1er trimestre de grossesse passé ?

Nausea does not occur in the same way in all women. We must then seek the treatment which will be the most effective in each. Here are some tips given by our expert, Rachel Halimi:

Hygien-dietetic rules

They must be applied as early as possible before any medication management.Associate them with other prescribed therapeutic treatments, if they are not effective.

- Hydrate yourself regularly and in small quantities outside of meals.

- Avoid having an empty stomach, taking meals frequently in small quantities (consume 4 to 6 snacks per day every 3 hours approximately).Eat as soon as you are hungry.NVGs can be accentuated if you are on an empty stomach.

- If possible, have breakfast in bed in the morning 30 minutes before getting up and prefer wholemeal bread or full cereals.

- Avoid having too full stomach.Who says eat regularly, does not prevent eating light.

- Avoid indigestible or too fatty food.Avoid tea, coffee, and carbonated drinks as well as spicy, vinegar or acidic foods.

- Try not to drink too much during meals and immediately before or after.Often drink small amounts of liquid during the day.Indeed, the solid/liquid mixture stimulates the activity of the stomach and amplifies the too full feeling.

- Eat hazelnuts and foods rich in protein and carbohydrates (wholemeal, complete pasta, etc.)

- Avoid lying on your back directly after the meal, make a little walk.

- Rest as soon as possible, do not hesitate to take naps.Sleep enough because fatigue increases nausea.

- Avoid stress sources and keep yourself.Psychic component and fatigue can influence NVG.

Homeopathy

Several homeopathic remedies, in the form of pellet tubes, can relieve your nausea.

-Vomica 9 CH: 5 granules to take 3 times a day, to be renewed as soon as the nausea is reappeared.

- SEPIA OFFICINALIS 9CH: 5 granules to take before getting up and 5 granules before breakfast, to help calm morning nausea and decrease fatigue

Acupuncture and acupressure

Acupuncture consists in stimulating specific points of the body using needles.Acupression uses the same principle by replacing the needles with a digitopression massage (massage with fingers).

Point P6 (point Neiguan) is a point located three fingers upstream of the wrist.Wearing an anti-nausea bracelet may therefore help you.It is a non -invasive ancestral technique, inexpensive, usable on demand.

Food supplements:

Vitamin B6 (or pyridoxine) can help calm nausea, the recommended dosage is 25 mg, 3 times a day.(Max dose: 200 mg/day).

Phytotherapy

Ginger is a root used for a long time in the treatment of nausea and vomiting. It can be found in the form of spice, tablets, infusion or drink.

Nausea maternov is a food supplement in the form of capsules whose dosage is 1 gram per day (which can be envisaged as a non -pharmacological option).

You can also enhance your dishes with fresh ginger.

Medical treatment

To date, unfortunately, there is no recommendation for the drug management of nausea and vomiting specific to the state of pregnancy in France.Management should avoid drug treatments as much as possible. However, when NVG affects quality of life and non -drug measures are ineffective, it seems necessary to set up an adapted therapy.

"We will prefer first intention either doxylamine, alone (Donormyl) or in association with vitamin B6 (Cariban®), then second intention Metoclopramide (Primperan®, Prokinyl®)" recommends Rachel Halimi.

Doxylamine is an antihistamine, having, in France, the marketing authorization for occasional adult insomnia. In the United States and Canada, it is marketed in association with pyridoxine (vitamin B6) under the respective names of Diglesis® (ex-Bendectin®) and Diclectin®.In these two countries, doxylamine associated with pyridoxin is the reference treatment against Nauées.It is recommended by the Society of Obstetricians and Gynecologists of Canada (SGOC).This combination of Doxylamine/Pyridoxine is the most studied treatment across the Atlantic, the profile of efficiency, side effects and teratogenic risks is the surest.

The very important impact of nausea on the future mother

Pregnant, nausea and vomiting are a real handicap, whether at home, at work, when you are invited to friends, to do the shopping ... Impossible to be quiet!Especially if they persist throughout pregnancy.Although they are completely normal, they prevent living a quiet pregnancy and can even lead to depression."Despite the number of women concerned, NVGs are still too often trivialized by the medical profession, since these symptoms are normal and do not affect the health of the mother and the baby" deplores our midwife, adding that inAs a result, they are very little studied in France.

However, if you suffer from it, never hesitate to talk about it around you or a professional health.
